The report must include facts vs. opinions. For example, the victim, John Smith complained of nausea vs. he looked like he had been drinking. Use the third person when writing the report. For example, On March 1, 2008, at 6:00 pm, Angie Gora, Summer Program Director was called by Desk Clerk, Abby Cole to respond to a medical emergency at the Fieldhouse. Upon arriving at the scene, Angie noticed a crowd gathered and a man holding his stomach, sitting on the floor. He was sweating. Angie approached the man and asked what kind of symptoms he was feeling and asked the man for his name. The man identified himself at John Smith and said, I feel faint and nauseated. Angie asked on a scale of one to ten the severity of his illness. He explained it was an 8. Since the man was coherent, Angie asked John if he felt like he needed an ambulance. John said, I would like some water please. Angie asked the man for his address and phone number for an incident report. She explained that because he still looked in poor condition, she should call an ambulance just to be on the safe side. She said, if the ambulance feels you are in good enough health, you can refuse to be transported. However, maybe they should make an assessment, just in case. John agreed and Angie asked someone to call 911. The fire department, Bemidji Ambulance Services and a Police Officer arrived within a few minutes. The ambulance service checked Johns heart rate and asked him some questions. John had continued to drink water. The EMT, Bill State told Angie that the man seemed like he just overheated. John got to his feet, toweled off, and decided to take it easy. He refused the ambulance service and got a ride home with a friend, Mike Walters.
